Notice: IMPORTANT HEALTH COVERAGE TAX DOCUMENTS - Alternate Reporting Options | Paperwork Burden Reduction Act
Employees eligible for minimum essential health coverage provided by the Afognak Native Corporation (“Afognak”) and its direct and indirect subsidiaries, partnerships, joint ventures and affiliates are entitled to a Form 1095-C, which serves as proof of the coverage we offered you. This statement was previously mailed to individuals; however, due to new regulations designed to reduce paperwork and administrative burden, we are no longer required to send paper copies via mail.
Electronic forms can be accessed through the Benefits Portal or directly from paperlessemployee.com/afognak. However, if you would like to request a copy of your Form 1095-C for your records, you may do so by emailing benefits@afognak.com or sending a request to Benefits Team at 737 Volvo Parkway, Suite 150, Chesapeake Virginia 23320. A paper copy will be mailed to the address on file within 30 days of receiving the request.
